Packages
containing radioactive materials must be checked immediately upon arrival
by Environmental Health & Safety. This procedure protects CSUN employees
and is required by regulation. In order to ensure that all packages are
checked it is essential to:

Clearly identify radioactive materials on Purchase Orders and Requisitions. |
|
|
Ensure that all redioactive materials are delivered directly to Central Receiving and nowhere else. |
| IN ORDER TO ACCOMPLISH THE ABOVE, PERSONS ORDERING RADIOACTIVE MATERIALS MUST COMPLETE THE PURCHASE ORDER OF REQUISITION STATING THE FOLLOWING: |

TO ENSURE THAT PACKAGES ARE DELIVERED TO CENTRAL RECEIVING, THE FOLLOWING ADDRESS MUST BE ENTERED AS THE "HEADER" OR "PRIMARY DELIVERY ADDRESS": |

THE ON-CAMPUS DELIVERY ADDRESS, (BLDG / DEPT / OFFICE), MUST BE ENTERED IN A TEXT (DESCRIPTION) FIELD. THE NAME OF THE PERSON(S) RECEIVING THE RADIOACTIVE ITEM(S) AND THEIR PHONE CONTACT SHOULD BE ENTERED THERE AS WELL. |

THE UNIVERSITY CORPORATION: The University Corporation must recognize, and forward to Central Receiving, the Purchase Order that contains the "Radioactive Material" warning. This is extremely important because of possible exposure to radioactive materials by CSUN employees, who will otherwise proceed to open the package. |
