If you are requesting a leave of absence:
I. Contact the Benefits Office
- We will discuss your options with you.

II. Complete an Employee Leave of Absence Request Worksheet
III. Medical Certification (medical/maternity leave only)
- Your physician must state the beginning and end date of a medical leave.
- It should list only the dates you will miss work.
- No diagnosis is necessary.
IV. Faculty Affairs Application for Maternity/Paternity Leave (if applicable)
When you return to work
- Stay in touch with your department during your leave. Communicate any leave changes (reduced or extended leave), and let them know when you expect to return.
- On your first day back to work, please bring medical certification of your readiness to work.
