What is Open Enrollment?
Every autumn, the CSU system allows employees to change their medical and dental coverage. Employees may enroll, change plans, or make changes to their own or family coverage.
If you are currently enrolled in either the Health Care or Dependent Care Reimbursement accounts (HCRA and DCRA), you must re-enroll during the Open Enrollment period.
If your current medical and dental coverage meets your needs, and you are not enrolled in HCRA or DCRA you do not need to take any action during the open enrollment period.
When is Open Enrollment?
September 15 - October 10, 2008
What if I miss the deadline?
If you miss the deadline, you will need to wait until next fall to change your coverage. If you do not re-enroll in HCRA or DCRA, your coverage will end.
Am I eligible?
You must be employed half time or more in an appointment that exceeds six months and one day. Temporary faculty (AY Classification) must have 6.0 units or more for at least one semester, including the current semester.

The co-payment for routine refraction (vision) exams to determine the need for vision correction and to provide a prescription for eyeglass lenses is being changed from $15.00 to $0.00.

How do I make changes to my Medical Plan?
- Print the Health Benefit Enrollment Form. Complete sections 1-4B, 6, 7, 9, 17, and 19 - 21; make sure to have Social Security Numbers for your spouse/domestic partner.
- Print and complete the Declaration of Health Coverage (HBD 12-A) Form .
- If you are adding a spouse, submit a copy of your Marriage Certificate; submit a copy of the approved Declaration of Domestic Partnership if adding a domestic partner.
- If you are adding a spouse, complete and submit a copy of the Gender Verification of Married Persons Form.
- Submit a copy of a Birth Certificate if adding a dependent child (under 23 years of age).
- Note: Return your enrollment form and appropriate copies to Benefits Administration,
University Hall, Room 165, by 10/10/08.
What are the Dental Plan changes for 2009?
Delta Dental (PPO) and DeltaCare USA continue to be Employer paid.
The new website address for CSU Delta Dental PPO/DeltaCare USA is http://www.deltadentalins.com/csu.
Dental implants are now available in the Delta Dental PPO Plan
Beginning January 1, 2009, implants will be covered under the prosthodontic benefit level (50 percent) and will be
subject to the plan’s prosthodontic limitation (once every five years), as well as the plan’s annual maximum and
deductible. As an alternative to partial and full dentures and bridges, dental implants and related procedures offer
enrollees an additional choice to replace lost teeth. Implant procedures include:
- Surgical placement of the implant (and removal, if necessary)
- Implant-supported prosthetics (crowns)
- Any repair and/or recementation
Employees are encouraged to obtain a pre-determination of benefits from Delta Dental to determine Delta
Dental’s and the employees’ share of the cost(s) relating to this service.
Please note, this additional benefit is not available on the DeltaCare USA plan.
Frequency of Certain Dental Services Now Based on Calendar Year!
Effective January 1, 2009, benefits for cleanings, examinations and bitewing x-rays will apply to a calendar year
frequency rather than a rolling 12-month period. This means that any time during the calendar year, Delta Dental
PPO and DeltaCare USA will pay for the following diagnostic and preventative:
- Two examinations
- Two cleanings
- Two sets of bitewing x-rays for children under age 18 or one set of bitewing x-rays for adults age 18 or
over.
When the new benefit period begins on January 1, 2009, any cleanings, examinations and/or bitewing x-rays that
were paid for on behalf of the employee and/or eligible dependents in 2008, will not affect the enrollee’s eligibility
to receive those same services in 2009.
For example: if an enrollee has a cleaning provided in November, 2008,
that cleaning will not count as one of the two cleanings provided under the plan in 2009, even if the enrollee’s first
cleaning in 2009 is less than six months from the November 2008 cleaning.

FlexCash
Can I be compensated if I wave CSUN benefit coverage?
- If you are covered by a non-CSU health plan, you may enroll in FlexCash. This benefit allows you to waive your CSU health plan and receive cash reimbursement. $128 (medical); $12 (dental); $140 (both medical and dental).

How do I enroll in FlexCash?
- Complete sections 1-6 and print the FlexCash Enrollment Form.
- Sign and date the form.
- Include a copy your insurance card, showing proof of insurance.

Dependant or Health Care Reimbursement Accounts
Are you missing an opportunity to put more money in your pocket?
- These programs allow you to pay for dependent care and/or health care out-of-pocket expenses with pre-tax dollars. Health Care Reimbursement Accounts (HCRAs) are designed to assist you in paying deductibles or co-payments with pre-tax dollars. Dependent Care Reimbursement Accounts (DCRAs) are designed to assist you in paying for child care or adult day care for dependent adults, also with pre-tax dollars.
- You may enroll for the first time during Open Enrollment.
- You must re-enroll in these programs each year.
- Administrative fee remains $1.00 per month for each plan.

How do I enroll/re-enroll in DCRA/HCRA?
- Access the HCRA/DCRA Enrollment Form. Print this form out and complete sections
1 - 5 and section 7.
- Sign and date the form.
