What is Open Enrollment?
Every autumn, CalPERS allows employees to change their medical and dental coverage. Employees may enroll,
change plans, or make changes to their own or family coverage.
If you are currently enrolled in either Health Care or Dependent Care Reimbursement Accounts (HCRA and DCRA), you must re-enroll during the Open Enrollment period.
If your current medical and dental coverage meets your needs, and you are NOT enrolled in HCRA or DCRA, no action is required.
When is Open Enrollment?
September 17 through October 12, 2007
What if I miss the deadline?
If you miss the deadline, you will need to wait until next fall to change your coverage unless you have a family status change. If you do not re-enroll in HCRA or DCRA, your coverage will end.
Am I eligible?
You must be employed half time or more in an appointment to exceed six months and one day. Temporary faculty (AY Classification) must have 6.0 units or more for at least one semester.
Medical Plan Changes
- 1. New Medical Plan options
CalPERS is offering two new Medical Plan options: Blue Shield NetValue (HMO) and PERS Select (PPO). These plans use a smaller panel of physicians than in the standard Blue Shield network and the PERS PPO; the advantage is that there is little or no premium to you.
- 2. Some Blue Shield Members have been automatically transferred to the Blue Shield NetValue option. If you are part of the group being transferred, you were notified of this transfer by mail and you will receive a confirmation letter during the week of September 17, 2007. Upon receipt of the letter, if you choose another plan, complete the Health
Benefit Enrollment Form and return to Human Resources, Benefits Administration, University Hall Room 165 by October 12, 2007.Press enter for that form.
- 3. PERS Select's new plan includes a provider network only in California.
- 4. HMO Plan Structure Changes
- Blue Shield, Blue Shield NetValue & Kaiser Permanente
- • No co-pays for most preventative care visits.
- • Co-pays will increase by $5.00 (from $10.00 to $15.00)
- • Co-pays for urgent care visits will be $15.00.
- • Annual out-of-pocket maximum will be $1,500 for individual and $3,000 for family. Pharmacy is not
included.

How do I make a change to my Medical Plan?
- • Press enter to access the Health Benefit Enrollment Form. Complete sections 1-4B, 6, 7, 9, 17, and 19 - 21; make sure to have Social Security Numbers for your spouse/domestic partner.
- • Submit a copy of your Marriage Certificate if adding your spouse; submit a copy of the approved Declaration of Domestic Partnership if adding a domestic partner.
- • Submit a copy of a Birth Certificate if adding a dependent child (under 23 years of age).

Dental Plans
Dental Plan Changes (change in plan name)
Delta Dental (PPO) and DeltaCare USA continue to be Employer paid.
DeltaCare PMI has been renamed DeltaCare USA.

How do I make changes to my Dental Plan?
- • Press enter to access the Dental
Plan Enrollment Form. Print this form and complete sections A, B, and D; make sure to have Social Security Numbers for your spouse/domestic partner.

FlexCash Plan:
- • If you are covered by a non-CSU health plan, you may enroll in FlexCash. This benefit allows you to waive your CSU health plan and receive cash reimbursement. $128 (medical); $12 (dental); $140 (both medical and dental).

How do I enroll in FlexCash?

- • Sign and date the form.

Dependant or Health Care Reimbursement Accounts:
- • These programs allow you to pay for dependent care and/or health care out-of-pocket expenses with pre-tax dollars. Health Care Reimbursement Accounts (HCRAs) are designed to assist you in paying deductibles or co-payments with pre-tax dollars. Dependent Care Reimbursement Accounts (DCRAs) are designed to assist you in paying for child care or adult day care for dependent adults, also with pre-tax dollars.
• You may enroll for the first time during Open Enrollment.
• You must re-enroll in these programs each year.
- • Administrative fee remains $1.00 per month for each plan.

How do I enroll/re-enroll in DCRA/HCRA?
