Academic Field Trip Transportation
IF TRAVELING BY CHARTER BUS OR VAN:
Be sure that the contract for the bus company is processed by the CSUN Purchasing Department to guarantee that the insurance and indemnification
requirements have
been met.
IF TRAVELING BY AIR:
All students participating in programs that require air travel must be informed in writing that participating in such program is voluntary. Air travel involves risks to personal safety, which could result in damage to property, injury, or death (Attachment G). Students participating in such travel must be informed in writing that the California State University assumes no liability for damage, injury or death occurring on such voluntary air travel and that students undertake such travel at their own risk.
All students participating in programs that require air travel shall be required to acknowledge that they have been informed of the risks of air travel required by such programs and to sign a statement certifying that they have been informed of the risks and undertake such air travel voluntarily with full knowledge of such risks. (Appendix I)
All students participating in programs, which require air travel, must sign the appropriate Air Travel Release and Hold Harmless Agreement.
IF FACULTY, STAFF AND/OR APPROVED VOLUNTEER EMPLOYEES WILL BE
DRIVING:
The Authorization to Use Privately Owned Vehicles on State Business must be filled out the first time this person drives on a field trip. This form is to be authorized by Department Head/Chair and filed in the department. This form must be renewed annually.
IF STUDENTS WILL BE DRIVING:
Identify student drivers and those students who will carpool with drivers. Each student driver must complete the Student Authorization to Operate Privately Owned Vehicle for Any University-Affiliated Program or Trip form. Be sure insurance information is complete before you countersign.This form is to be authorized by the Department Head/Chair and filed in the department.
